| Step 3 | Choose Download Health Card Option from the list | |
| Step 4 | It will show you a message that your mobile no is not rgistered. Ignore this message. | |
| Step 5 | It will ask you whether your policy is an Indivisual policy or Corporate Policy? Choose Corporate Policy. | |
| Step 6 | It will show you options choose "EmpID/PolicyNo". | |
| Step 7 | It will ask you to enter Policy Number. Enter "2816/74292014/01/000". | |
| Step 8 | Then It will ask you to enter your employee Id which is your Membership Id provided by the society. For example 2400400001. | |
| Step 9 | Then It will show you all the members(including dependents) attached to your employee id and ask Please select from the following to download the Health Service Card. Choose any one preferably Primary member to download your health card. |
